Yes, for your convenience CMC and Rite Aid partner to provide a Meds-to-Beds service that includes FREE delivery of medications to your hospital room (9 AM–8 PM), the Discharge Hospitality Lounge or to your home Mon-Fri (9 AM–12 PM). Please ask your nurse about the Meds-to-Beds form.

Due to privacy practices, medical records cannot be requested online. You must complete and submit an Authorization to Release form to the CMC Health Information Management/Medical Records department. This form, and additional information about how to request copies of your medical record can be found by visiting our Medical Records page. You may also call the office at 603.663.6516 (M-F).

Contact our Radiology Imaging Department at 603.663.6449 or register for CMC's Patient Portal and download an Authorization Release form.
